Q. How do I book an appointment?

A. As a new client to the practice please call 857.330.6565 to speak with a booking agent and register your insurance.

Q. What do I need to bring to my first appointment?

A. Please bring a photo ID, insurance card, co-payment and current medication list. You may also bring any past records that you feel are beneficial to your treatment. 

Q. What can I do to prepare for my first appointment?

A. Lakeside Mental Health asks new clients to arrive 15-20 minutes early for their appointment to complete intake paperwork. If you wish these forms are located online under the New Clients Forms tab. You may complete the forms at home and bring them with you. If you have questions your provider can answer them in person at your appointment. 

Q: What do I do if I need to reschedule my appointment or weather does not allow me to make it to my appointment safely?

A: Please call the office number to notify your provider. If you know that weather will be unsafe on the day of your appointment you can email your provider before hand and they will contact you to reschedule. If email is not an option please call the office. If you are a new client please call the 800 number to reschedule.
